extension not working on Linux and Mac OS

I tried to install and use this extension together with the xmldap firefox identity selector.
 
I could install on both Linux and Mac OS, but it will not work on any of them.
 
On Linux it could not locate it's jar file, IdentitySelector.jar, due to filename case (camel case on file system, all lower case in the code). Renamed the file, the next error is the same as on Mac OS. Preferences dialog will not open, just a tiny (almost invisible) blank window.

This extension was/is not intended to work on other operating systems than Windows. I added a targetPlatform element to the install.rdf to express this restriction

Here is the exact message for the filename case issue on Linux:

Firefox can't find the file at file:///home/xxx/.mozilla/firefox/fow0hki5.default/extensions/{5a6eeede-5d1c-40ee-9f81-dad96f32a573}/chrome/identityselector.jar!/content/identityselector.xul.

Also attached a screenshot of the error window. The original size of the window was tiny, probably 1 pixel, I had to resize it. Not sure why.

Attached a screenshot of the tiny window. This show in both cases, first with the filename error and then instead of the preferences window.
